Open a new image 500 X 500 transparent.

Your locket won't be this big but this gives you room to work.

Select your Ellipse preset shape.

Set your color palette with white as the foreground and light grey as the background.

Change the background to gradient with the settings shown below.

In the preset shape options UNcheck vector and set the line width to 1.
Make an ellipse on your image.
Apply Inner bevel using the following settings.

In your layer palette, duplicate your layer TWO times for a total of THREE identical layers.
Rename them Top, Rim, Bottom as shown below.

Still in your layer palette, turn OFF the top and bottom layers
so that only the rim layer is on and active.

Select all (CTRL + A)
Click on your ellipse using your selection tool.

Selections > Modify > Contract > 10

Press delete.

DO NOT DESELECT.

Make a new layer named LENS1

Apply Cutout with the settings shown below.

Make a new layer named "LENS2"

Apply the SAME cutout but change the horizontal and vertical from -5 to 5.

Deselect.

Click on the RIM layer
Open the picture or tube that you would like to use in your locket, resize it so it fits. (its ok if it sticks out a tad) and paste it as a new layer.
This new layer should be located just above the rim in the layer palette.

Activate the rim layer again. Use your magic wand tool and click on the INSIDE of the ellipse.

Click Selections > Invert.

Select the tube layer and press delete. This will delete any part of the tube that sticks out past the lens.

In your layer palette, turn the BOTTOM layer on (the top layer is still off) and merge visible,

Turn the top layer on and use the deformation tool. Hold the SHIFT and CTRL keys at the same time.
With your mouse grab the node (little square) along the right edge and push UP and IN at the same time until you get the locket cover the way you want it.

**HINT**

if the WHOLE locket cover moves, you didn't do it right. The left edge of the locket cover should not move at all.

When you have the locket cover the way you want it, apply the deformation.
Merge visible. copy and paste as a new image. this will get rid of any unneeded transparency.

Add a drop shadow, settings are your choice. Add a background layer if you like.

VOILA. You're done.
